Token-based Surveys

The Provost's Technology Group offers token-based surveys for upward administrative evaluation and other evaluation opportunities to allow anonymous feedback. While all data collected are subject to public information legislation, only the comments are collected; tokens are randomly assigned and therefore not connected to any individual.

Any open surveys are listed below. To participate, you'll need to enter the token string you received as a part of your invitation to provide feedback; the token was delivered to you either electronically or in print, but all delivery systems are randomized to provide anonymity to the extent allowed by law and university policy.

Each token is good for only one session. If you begin an evaluation or survey, you should complete the survey in a single setting; the token will expire when your session closes or times out.

To see a preview of a survey, simply "click" on the survey title. You may preview any survey without enabling your token.

In the box below, enter the token string exactly as it appears in your token document, including dashes or other symbols to begin your survey. Each token is assigned to a specific survey; the survey matching your token will open automatically. Answer each question as you work through the survey pages. When you finish, you'll be given an opportunity to review your answers before being asked to use the SUBMIT button to close your survey.
